Full-time position where you will serve as the primary point of contact for elderly individuals, individuals with disabilities, and their families seeking information and assistance regarding available resources and support services.
Conduct comprehensive assessments to identify the specific needs and preferences of individuals, considering their unique circumstances, abilities, and goals.
Provide personalized guidance and counseling to individuals and families, offering information on available options, benefits, and community programs.
Coordinate and facilitate access to services, including assisting individuals with completing applications, navigating eligibility criteria, and connecting them with appropriate providers.
Create and uphold an extensive database of community resources encompassing various domains such as healthcare providers, housing options, transportation services, support groups, eviction prevention/intervention resources, foreclosure prevention, nutrition assistance programs, and other relevant organizations.
Perform outreach and educational events to raise awareness about available resources and promote the agency's services (e.
g.
, presentations, radio tips, health fairs, etc.
)
Qualifications:
BA/BS/BSW degree preferred with 3-5 years of demonstrated work experience in insurance or related field, particularly with seniors and younger persons with disabilities or in lieu thereof, any combination of work experience and education that can meet the obligations of general duties above.
Excellent interpersonal and communication skills, with the ability to engage effectively with clients, families, and professionals from diverse backgrounds.
Strong organizational and time management skills, with the ability to prioritize tasks and handle multiple responsibilities effectively.
Proficiency in using computer systems, databases, and relevant software applications, (e.
g.
, Word, PowerPoint, Publisher, Excel, TEAMS, etc.
) for recordkeeping and reporting.
Familiarity with relevant laws and regulations related to aging and disability services.
Vehicle and valid driver's license required as this position may require travel in our 41-town area.
Second language a plus.
Ability to lift/carry 5-10 lbs.
